NY Residents who care for their loved one can receive wages and benefits thanks to a program called Consumer Directed Personal Assistance Program (CDPAP).
CDPAP is a New York State Medicaid program that not only allows people to get help with daily activities and medical conditions, but also to choose and pay their own caregiver. Caregivers under this program are not required to have any license or certification.
To be covered by the CDPAP program, patients need to have a social and nursing assessment initiated by their doctor. After that, a nurse assessor determines whether they can appropriately participate in CDPAP, and if so, recommends the amount, frequency and duration of services.
Caregivers paid through Ultimate Care get competitive pay and bonuses, overtime pay, essential coverage health plan (FREE for individuals) with optional low-cost vision and dental insurance, paid vacation, sick time and holidays, and 401(k) with company match, Employee Discount Program (Tickets at work), FREE T-Mobile cell phone (min 30 hours per week to qualify), Employee Assistance Program, Wage Parity Benefits.
Must meet all this criteria: be eligible for Medicaid, have a stable medical condition, need some or total assistance with one or more personal care services, home health aide services, or skilled nursing task. They or the person acting on behalf of them must also be willing to make informed decisions with regards to their care.
Are Not Eligible: Legal spouses, parents caring for a child under 21, power of attorney, and healthcare proxy.
We are a fully licensed and insured (with full coverage insurance) New York home care agency.
We provide personalized reminder calls in advance of assessments. We also conduct house calls for homebound patients and follow up to ensure patient satisfaction.
No obligation or no long-term contracts. You are free to cancel anytime, just give us a 24 hour notice.
We have a Dedicated Quality Assurance Team to ensure services are meeting quality standards. We have a process for our patients and caregivers (even if they're family or friends!) to share their grievances and for us to take the proper measure to improve their level of satisfaction.
We help you navigate enrollment and insurance, coordinate directly with your doctors, participate in patient-related conference calls, explain all available care options carefully and explicitly. We make sure you're proceeding from a place of clarity.
We are a Fiscal Intermediary for CDPAP. We provide one-on-one guidance for patients applying for the NY Consumer Directed Personal Assistance Program (CDPAP).
Also, when the transition within the health care system feels confusing and difficult, our staff is here to assure smooth and safe transitions between insurances, physicians, levels of care, and anything else our patients may need.
At Citadel Home Care, the health and safety of our clients is our top priority. In light of COVID-19, it is our goal to provide peace of mind about the measures we are taking to prevent the spread of the virus and protect our patients in these uncertain times.
Our team has implemented a reporting policy to ensure that all caregivers are able to identify risk factors and prevent the spread of the virus between patients. Our office staff has also been trained to identify caregivers who are at risk and will not send them to work.
While providing caregivers with additional supplies, we have also implemented additional hand washing procedures. Each caregiver is required to wash their hands prior to entering a patient’s home as well as once per hour while providing care for the patient.
For patients who would like to reduce the number of caregiver visits to their home, live-in care is an excellent alternative.
Live-in caregivers remain at the patient's home to reduce contact with others and provide all necessary care. Live-in care services provide the benefits of reducing risk for accidents at home, decreased chances of illness or injury, and increased socialization.
