The NHTD Medicaid Waiver Program is designed to help individuals with disabilities or seniors avoid institutionalized care by receiving services in their own homes or communities.
These services include personal care aides, nursing services, and therapeutic supports.
That plan is developed with the client, the service coordinator, and the service provider.
There's flexibility in choosing where and with whom clients receives care.
Individuals 18+ years old with a disability or seniors over 65 who qualify for Medicaid.
Must be medically eligible for nursing home level of care but wish to remain in a home or community setting.
Service coordinator refers eligible individual to NHTD.
A care plan is developed that addresses the individual’s health needs.
Service coordinator plays a key role in finding a competent and approved NHTD home care provider.
Once a provider is selected, the care plan is put into action with the support of a competent home care team.

At Citadel Home Care, the health and safety of our clients is our top priority. In light of COVID-19, it is our goal to provide peace of mind about the measures we are taking to prevent the spread of the virus and protect our patients in these uncertain times.
Our team has implemented a reporting policy to ensure that all caregivers are able to identify risk factors and prevent the spread of the virus between patients. Our office staff has also been trained to identify caregivers who are at risk and will not send them to work.
While providing caregivers with additional supplies, we have also implemented additional hand washing procedures. Each caregiver is required to wash their hands prior to entering a patient’s home as well as once per hour while providing care for the patient.
For patients who would like to reduce the number of caregiver visits to their home, live-in care is an excellent alternative.
Live-in caregivers remain at the patient's home to reduce contact with others and provide all necessary care. Live-in care services provide the benefits of reducing risk for accidents at home, decreased chances of illness or injury, and increased socialization.

We conduct a home assessment and work with the service coordinator to develop a Person-Centered Service Plan (PCSP).
