NH Residents who care for their loved one can receive wages and benefits thanks to a program called Choices For Independence Program (CFI).
CFI is a New Hampshire State Medicaid program and administered by the Bureau of Elderly and Adult Services that not only allows people to get help with daily activities and medical conditions, but also to choose and pay their own caregiver. Caregivers under this program are not required to have any license or certification.
Patients have to be approved for Medicaid CFI and have a social and nursing assessment initiated by their doctor. After that, a nurse assessor determines whether they can appropriately participate in CFI, and if so, recommends the amount, frequency and duration of services.
Caregivers hired by and paid through Eminence Home Care get competitive pay and bonuses, overtime pay, essential coverage health plan (FREE for individuals), and 401(k) with company match, Employee Assistance Program, Wage Parity Benefits.
Must meet all this criteria: be eligible for Medicaid, medically qualify for the level of care provided in nursing facilities, need some or total assistance with one or more personal care services, home health aide services, or skilled nursing task.
We are a fully licensed and insured (with full coverage insurance) New Hampshire home care agency.
We provide personalized reminder calls in advance of assessments. We also conduct house calls for homebound patients and follow up to ensure patient satisfaction.
We have a Dedicated Quality Assurance Team to ensure services are meeting quality standards. We have a process for our patients and caregivers (even if they're family or friends!) to share their grievances and for us to take the proper measure to improve their level of satisfaction.
We help you navigate enrollment and insurance, coordinate directly with your doctors, participate in patient-related conference calls, explain all available care options carefully and explicitly. We make sure you're proceeding from a place of clarity.
We are a Fiscal Intermediary for CDPAP. We provide one-on-one guidance for patients applying for the NY Consumer Directed Personal Assistance Program (CDPAP).
When the transition within the health care system feels confusing and difficult, our staff is here to assure smooth and safe transitions between insurances, physicians, levels of care, and anything else our patients may need.
At Citadel Home Care, the health and safety of our clients is our top priority. In light of COVID-19, it is our goal to provide peace of mind about the measures we are taking to prevent the spread of the virus and protect our patients in these uncertain times.
Our team has implemented a reporting policy to ensure that all caregivers are able to identify risk factors and prevent the spread of the virus between patients. Our office staff has also been trained to identify caregivers who are at risk and will not send them to work.
While providing caregivers with additional supplies, we have also implemented additional hand washing procedures. Each caregiver is required to wash their hands prior to entering a patient’s home as well as once per hour while providing care for the patient.
For patients who would like to reduce the number of caregiver visits to their home, live-in care is an excellent alternative.
Live-in caregivers remain at the patient's home to reduce contact with others and provide all necessary care. Live-in care services provide the benefits of reducing risk for accidents at home, decreased chances of illness or injury, and increased socialization.
